Two permanent direct hire openings are available for Ambulatory Clinical Pharmacy Specialists within a well-established healthcare system in Kent County, Delaware. This role is ideal for Board Certified pharmacists specializing in Ambulatory Care, Pharmacotherapy, Cardiology, or Geriatrics seeking to deliver advanced clinical pharmacy services in an outpatient setting.

Schedule:

  • Monday through Friday
  • 8:00 AM to 4:30 PM or 8:30 AM to 5:00 PM
  • No weekends or holidays
  • Role Overview:
    As an Ambulatory Clinical Pharmacy Specialist, you will provide comprehensive medication management services as a credentialed and privileged member of the healthcare team. Under Collaborative Practice Agreements, you will assess, monitor, and document therapeutic plans to optimize patient outcomes, improve medication access, and reduce costs. Your expertise will support chronic disease management, specialty medication coordination, and comprehensive medication management across the ambulatory network. This leadership role involves collaboration with providers, nursing staff, and multidisciplinary teams to enhance quality and safety.

    Key Responsibilities:

  • Conduct medication reconciliation, counseling, laboratory monitoring, and disease management during patient visits
  • Collaborate with providers to implement evidence-based medication interventions
  • Manage referrals with documented communication to clinicians
  • Coordinate prior authorizations, financial assistance, and benefit investigations
  • Oversee specialty medication initiation and monitoring
  • Provide drug information and education to patients and healthcare team members
  • Precept pharmacy students, interns, and residents
  • Participate in quality improvement projects, committees, research, and policy development
  • Document all interventions and report outcomes in electronic health records
  • Qualifications:

  • Active Delaware Pharmacist License or willingness to quickly obtain
  • Board of Pharmacy Specialties (BPS) Certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (mandatory)
  • Minimum one year of clinical pharmacy experience, including residency
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD preferred)
  • Experience with Collaborative Practice Agreements and specialty medication management strongly preferred
  • Ambulatory care residency or PGY1/PGY2 preferred
