Remote Pharmacist Opportunity – Medication Therapy Management (MTM) Consulting | Multi-State Licensed | 13-Week Contract with Extensions

This remote role focuses on telehealth MTM consulting, supporting a rapidly expanding telepharmacy service currently serving Louisiana, Mississippi, and Texas with plans to extend to 30–40 states by October. The primary hub is located in Louisiana, offering increased full-time work opportunities for candidates licensed there along with other states. The contract is initially for 13 weeks with potential extensions, aligning with onboarding surges scheduled for November through January/February, and subsequent waves in April, May, and July.

Key Responsibilities:

Conduct virtual patient consultations via phone or video to perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs).

Address medication side effects, answer therapy-related questions, and promote medication adherence.

Evaluate prior authorization requests, formulary exceptions, and step-therapy limits based on clinical guidelines and safety standards.

Collaborate with providers and care teams via secure messaging or phone to recommend formulary-aligned, cost-effective alternatives.

Perform proactive clinical outreach to identify and close gaps in care such as statin use and diabetes monitoring.

Support formulary translation into PBM claim adjudication systems and review complex claims data.

Ensure all telepharmacy activities comply with HIPAA, state telepharmacy regulations, and client contracts.

Qualifications & Desired Experience:

Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree.

Active, unrestricted pharmacist license in one or more states; multi-state licensure highly preferred.

Minimum 2 years clinical pharmacy experience; PBM, managed care, or telehealth clinical environment expertise strongly preferred.

Solid knowledge of chronic disease management, drug utilization review, and formulary processes.

Proficient with telehealth platforms, electronic health records (EHR), and PBM claims systems.

Excellent verbal and written communication skills for patient and provider interactions.

Preferred Multi-State Licenses Include (but not limited to): Alaska, Arkansas, California, DC, Delaware, Hawaii, Indiana, Maine, Maryland, Massachusetts, Montana, New Hampshire, North Dakota, Oklahoma, South Dakota, Utah, Vermont, Virginia, West Virginia, Wyoming, and Louisiana (hub).
