Pharmacist – Long-Term Acute Care (LTAC)
Location: Marin County, CA
Employment Type: Part-Time (Flexible schedule with potential for Full-Time conversion)

This role offers a unique opportunity to provide clinical pharmacy support in a Long-Term Acute Care (LTAC) setting within a smaller, patient-centered hospital environment. Ideal candidates are pharmacists eager to maintain or develop clinical expertise while working closely with interdisciplinary teams to improve patient outcomes. The position allows for flexibility in scheduling and the potential to grow into a full-time role.

Responsibilities:

  • Review and accurately verify medication orders specific to the LTAC population
  • Support clinical pharmacy practices, including pharmacokinetics and anticoagulation monitoring where applicable
  • Assist with medication dispensing and manage automation workflows using Pyxis
  • Collaborate with physicians, nurses, and other healthcare professionals to optimize therapeutic plans
  • Ensure safe, effective medication management tailored to medically complex patients
  • Qualifications:

  • Bachelor of Science or Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy
  • Active, unrestricted California Pharmacist License in good standing
  • Preferred Experience:

  • 2–3 years of retail pharmacy experience with an interest in transitioning to or sustaining clinical/hospital pharmacy practice
  • Prior inpatient or hospital pharmacy experience
  • Knowledge and experience in TPN preparation and monitoring
  • Familiarity with pharmacokinetics and anticoagulation management
  • Current BLS and ACLS certification preferred
