RPh on the go is searching for Pharmacist. The ideal candidate will have a year or more LTC /retail experience.
Shift: M-F, Mid Shift (11am - 7:30pm). 32 hours/week
Key Responsibilities:

Medication Management:
- Review and analyze patient medication profiles to ensure appropriate, safe, and effective medication use.
- Monitor and evaluate medication regimens for therapeutic efficacy, potential side effects, and drug interactions.
- Recommend adjustments to medication therapy as needed, based on patient response and clinical guidelines.

Patient Care:
- Conduct medication therapy management (MTM) and medication reconciliation for new admissions and periodic reviews.
- Provide drug information and education to patients, families, and caregivers on proper medication use and potential side effects.
- Collaborate with healthcare teams, including physicians, nurses, and other staff, to optimize patient care plans.

Compliance and Documentation:
- Ensure compliance with federal, state, and local regulations regarding medication management and documentation.
- Maintain accurate and up-to-date records of medication orders, changes, and patient responses.
- Assist with audits and inspections to ensure adherence to regulatory standards.

Quality Assurance:
- Implement and monitor quality improvement initiatives related to medication use and patient safety.
- Develop and review policies and procedures related to medication management and pharmacy services.

Education and Training:
- Provide training and support to facility staff on medication-related topics, including new drug therapies and best practices.
- Stay current with advancements in pharmacotherapy and long-term care practices through continuous education and professional development.

Pharmacy Operations:
- Oversee the procurement, storage, and dispensing of medications within the facility.
- Ensure proper handling and disposal of medications, including controlled substances.
- Coordinate with pharmacy vendors and suppliers to ensure timely and accurate medication
Required:
- Pharmacy experience.
- LTC experience a plus.
- Washington Pharmacy license
