Soliant Health is seeking a Clinical Pharmacist I to join our team in the beautiful Coastal Oregon area. This is a unique opportunity to live and work in a picturesque location, known for its stunning coastline, outdoor recreational activities, and vibrant community.

As a Clinical Pharmacist I, you will be responsible and accountable for providing a broad range of inpatient and outpatient pharmaceutical care services. You will ensure safe, effective, and prompt medication therapy, supporting both centralized and decentralized services. You will collaborate with a multidisciplinary team to monitor, assess, and provide clinical interventions, ensuring that medication use is aligned with patient care needs, evidence-based practices, and regulatory standards.

Key Responsibilities:

- Provide comprehensive pharmaceutical care to patients with diverse disease states in both inpatient and outpatient settings.
- Collaborate with healthcare providers to deliver optimal medication therapy and clinical interventions.
- Support centralized and decentralized pharmacy services to enhance patient care.
- Ensure compliance with established policies, protocols, and regulatory standards.

Qualifications:

- Graduation from an accredited College of Pharmacy with a Doctor of Pharmacy (Pharm.D.) degree preferred or B.S. Pharm degree
- Current Oregon Pharmacist license required
- American Heart Association Basic Life Support (BLS) certification required
- American Heart Association Advanced Cardiovascular Life Support (ACLS) certification (must be obtained within 6 months of hire)
- A minimum of 3 years of relevant hospital experience preferred
