Job Title: Inpatient Hospital Pharmacist
Location: Winamac, IN
Shift: 30 hours/week – Full time

Job Description:
RPh on the Go is seeking a dedicated and highly knowledgeable Inpatient Hospital Pharmacist to join our clinical team in Indiana. The Inpatient Pharmacist will be responsible for the safe preparation, dispensing, and monitoring of medications for hospitalized patients. You will work closely with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes through safe and effective medication therapy.

Key Responsibilities:
•    Review, verify, and dispense inpatient medications based on physician orders and clinical protocols.
•    Provide clinical pharmacy services, including medication therapy management, therapeutic drug monitoring, and pharmacokinetic dosing.
•    Collaborate with the healthcare team to develop and implement individualized medication plans for patients.
•    Monitor patient responses to drug therapies and adjust medications as needed to ensure optimal efficacy and safety.
•    Educate patients, caregivers, and hospital staff on proper medication use, potential side effects, and drug interactions.
•    Participate in multidisciplinary rounds and provide clinical expertise on pharmacotherapy decisions.
•    Oversee and manage pharmacy technicians in the preparation and dispensing of medications, including sterile compounding.
•    Ensure compliance with all hospital policies, federal and state regulations, and accreditation standards (e.g., Joint Commission).
•    Maintain accurate and up-to-date patient medication profiles using electronic health records (EHR) and pharmacy management software.
•    Ensure proper storage, handling, and inventory management of medications, including controlled substances.
•    Participate in quality improvement initiatives and hospital committees focused on improving medication safety and patient care.

Qualifications:
•    Doctor of Pharmacy (Pharm.D.) from an accredited institution.
•    Active and unrestricted Indiana Pharmacist License or eligibility to obtain licensure before start date.
•    1-2 years of experience in a hospital or clinical pharmacy setting preferred.
•    Strong knowledge of inpatient pharmacotherapy, drug interactions, and clinical decision-making.
•    Proficiency with pharmacy management systems and electronic health records (EHR).
•    Excellent communication and collaboration skills with a focus on patient-centered care.
•    Ability to work effectively in a fast-paced environment with high attention to detail.
