A full-time Medication Therapy Management (MTM) pharmacist opportunity is available for a 13-week contract to hire in Zachary, LA. This position focuses exclusively on specialty MTM services and is not related to remote surge coverage. Candidates can work onsite or remotely but must hold a valid Louisiana pharmacist license and maintain at least five additional licenses from a specified list of states—California license being critical.

Key Responsibilities:

  • Prepare and dispense medications according to physician orders and established protocols.
  • Interpret and accurately transcribe verbal and written prescriptions into computerized patient medication profiles.
  • Maintain comprehensive, accurate pharmacy patient medication records.
  • Dispense pharmaceuticals safely and issue controlled substances adhering to legal requirements.
  • Monitor drug therapy regimens for contraindications, interactions, allergies, and dosing appropriateness.
  • Detect, report, and document adverse drug reactions promptly.
  • Provide clinical consultations, recommend cost-effective therapeutic alternatives, and clarify orders with medical staff.
  • Participate in quality improvement, medication use reviews, and associated documentation and data collection.
  • Supervise pharmacy support personnel, verify daily activities of pharmacy technicians, and participate in performance appraisals.
  • Conduct medication storage area inspections and ensure timely removal of outdated drugs.
  • Professionally handle telephone and in-person pharmacy requests.
  • Qualifications:

  • Current licensed pharmacist in Louisiana.
  • Experience in retail or hospital pharmacy settings.
  • Licensure in Louisiana and at least five additional states from the following (candidate must accumulate a minimum of ten states within 18 months):  California (critical), Indiana, North Carolina, South Carolina, Illinois, New Jersey, Iowa, New Hampshire, North Dakota, South Dakota, Utah, Washington, West Virginia, Arkansas, Kansas, Maine, Montana, Oklahoma, Virginia, Wyoming.
