Duties:
As a valued member of our team, you will:

• Review, interpret, and evaluate all medication orders for patients.
• Maintain patient medication profiles in the EMR (Electronic Medical Record) system.
• Consult with and advise medical, nursing, and other healthcare providers regarding optimal medication therapy.
• Routinely monitor patients' total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities.
• Actively participate in code blue situations.
• Perform and document Clinical Intervention activities such as recommendations on drug product selection, dosage adjustments, and appropriate drug therapy.
• Obtain medication histories of selected patients and communicate pertinent information to the appropriate healthcare provider.
• Prepare and verify the appropriate preparation of medications to be dispensed using the correct technique.

Requirements:

• BS or PharmD degree.
• Registered Pharmacist with the Ohio Board of Pharmacy in good standing (RPH).
• Minimum of 1 year of experience as a Pharmacist in an acute care setting.
