Position: Clinical Pharmacist
Contract Type: 13-week contract with potential for permanent hire
Shift: 1st shift, Monday-Friday, with every 4th weekend
Hours: 10:00 AM 7:00 PM
Job Overview: We are seeking a dedicated and experienced Clinical Pharmacist for a 13-week contract at a hospital in Blue Ash, OH, with potential for permanent placement. This role will support the hospital during a staffing shortage and offers the opportunity to transition to a full-time position if you re a great fit!
Key Responsibilities:

• Verify orders to ensure accurate dispensing of medications.
• Perform medication reconciliation to avoid discrepancies.
• Conduct Warfarin and Vancomycin consults for appropriate dosing and monitoring.
• Provide renal dosing adjustments for patients with kidney impairment.
• Ensure IV medication compatibility for safe patient care.

Requirements:

• Clear and active pharmacist license in the state of Ohio.
• At least 2+ years of pharmacist experience.
• Experience in an inpatient pharmacy setting is required.
• Familiarity with Wellsky EMR is preferred but not required.
